The Manticore left Earth orbit with a Klingon efficiency expert 
from the Federation Council onboard to observe and offer counsel 
to Sovak and McFly.  He messed with the ship, including changing 
our course to the middle of nowhere, blocking access to Bridge 
commands and Aux. Control, with a door fused shut.  Halting the 
ship and performing a cold shutdown of the warp core, he declared 
the examination to begin.  What no one could plan for was the sudden 
appearance of a wormhole nearby emitting a strange gravitation pull.  
Even with the Klingon trying to help out, the ship gets pulled into the 
wormhole, setting off a number of internal overloads/explosions and 
killing the Klingon.  As Sickbay deals with an overload of wounded, 
Engineering is able to get the warp engines online, but the ship is now 
stranded in some sort of null space with no stars for navigation and no 
sensor beacons to connect to.
